Funded by a grant from Providence, this is a partnership between Outside In, Rosewood Family Health Center, and the Coalition of Community Health Clinics. Outside In is providing outreach medical services in Clackamas County, and making primary care home referrals to eligible Clackamas residents. These referrals, managed by the Coalition, are assigned to Outside In and Rosewood Family Health Center.

To request a primary care medical home for a Clackamas client
Does the client fit all three guidelines?
Income under 200% FPL
Uninsured
Residing or homeless in Clackamas County
Yes!
They fit all three guidelines!
- Fill out this Clackamas Primary Care Referral form for each client.
- Fax it to the Coalition: Fax # 503-548-4849
- Let the client know they’ll hear from the Coalition within 2 weeks.
Please also submit a form for each child being referred.
No
They do not fit one of the guidelines:
- Find an alternate clinic by using the Coalition Clinic Search
This does not guarantee the patient will receive a primary care home, but acute care needs can be addressed through one of the coalition clinics listed on the website.
