A Learning Collaborative for Member Clinics

We help our member clinics remove barriers so all people can achieve their best health. Our learning collaborative – called H.E.A.L. Together – connects our coalition member clinics through shared learning, advocacy, and connection to advance health equity.

H.E.A.L. Together = Health. Equity. Advocate. Learn.

Through H.E.A.L. Together, staff and providers in our member clinics get support to:

  • Learn to advance health equity. We share and implement strategies to address health disparities and improve care for underserved communities.
  • Advocate for systemic change. We learn how to engage with decision-makers, influence policies, and secure funding for essential services.
  • Build health equity connections. We build relationships between safety net clinics to share best practices, problem-solve, and set collective priorities.

Ways to Get Involved

We host quarterly trainings on health equity, culturally and linguistically responsive care, emergency preparedness, policy advocacy, and more. Community Health Worker continuing education units (CEUs) are available.

Training Topics

REALD/SOGI Training and Technical Assistance

CCHC offers training and technical assistance on REALD (race, ethnicity, language, and disability) and SOGI (sexual orientation and gender identity) data collection.

This comprehensive program is designed to help safety-net clinics,  frontline healthcare staff, and community health workers (CHWs) strengthen health equity and inclusive care through improved demographic data collection.

Tobacco Prevention

CCHC equips member clinics staff with culturally tailored, evidence-based training to prevent and reduce commercial tobacco use—especially in communities of color targeted by decades of industry marketing.
 
Our commercial tobacco prevention, intervention, and education training helps outreach, intake, nursing and other staff engage patients with practical tools that advance equity and create healthier environments free of commercial tobacco.

Certified Community Health Worker Training

CCHC provides critical training and skill development for community health workers (CHWs), ensuring they are equipped to navigate complex health and social service systems on behalf of under-resourced communities.

CCHC provides workshops and training sessions specifically for community health workers, covering topics related to system navigation, Social Determinants of Health, public health, working in health care teams, social support navigation, and more.
