Partnering for Health Equity: Community Collaborations

CCHC partners with community-based organizations, non-member clinics, advocacy groups, and grassroots movements to advance health equity across the Portland metro region and beyond.

We believe everyone should have the opportunity to achieve health and well-being. We’re committed to making this a reality by working with our partners who know their communities best.

Whether you’re a direct service provider, housing advocate, culturally-specific community hub, or nonprofit navigating health systems — we offer training, resources, and support for your staff, community health workers, and your community members.

Together, we bridge gaps and build systems that work better for the communities we serve.

Our Services

To promote health equity and foster collaboration between community health clinics, community-based organizations, and other community partners, we offer the following types of support.

Why Partner With CCHC

As a trusted connector across clinical and community spaces, we:

  • Offer real-time support for community needs—from housing to health insurance to healing.
  • Connect organizations to health equity training, CHW certification, and emergency preparedness tools.
  • Amplify community voices in policy spaces, ensuring underrepresented needs are heard.
  • Build bridges between community groups and the clinics that serve them.

Our Partners

We partner with all kinds of community-based organizations. Here are some of our most active partnerships:

Unite Us / Connect Oregon
We participate in the Unite Us system and utilize the Connect Oregon platform to securely connect individuals to services in their community that meet their needs. Use this form to connect someone with resources.

Join
We partner with JOIN’s community agencies to help individuals and families transition from homelessness to stable housing. Together, we provide wrap-around services including case management, mental health, health advocacy, and peer support services to individuals and families who are new to housing.

CareOregon
We partner with CareOregon to help individuals and families enroll in the Oregon Health Plan (OHP). Through this collaboration, our health navigators and CareOregon’s Connect to Care Team work together to ensure accessible, multilingual OHP assistance—no appointment needed.

Oregon Public Health Institute
We partner with OHPI’s Right from the Start program to help community health workers and clinic partner staff improve and advocate for individual and community health outcomes by understanding and addressing the root causes of commercial tobacco use.

Greater Northwest Community Health Collective
We partner with Greater Northwest Community Health Collective to provide a 90-hour CHW training, tobacco workshops, and community health and wellness fairs for Black, Indigenous, and People of Color.

The Rosewood Initiative
We partner with The Rosewood Initiative to co-host our annual community health fairs, bringing clinic services directly to the community.

Community Services Network PDX
We partner with The Community Service Network PDX to co-host our annual community health fairs, bringing clinic services directly to the community.

Immigrant and Refugee Community Organization
We partner with IRCO to co-host our community health fairs and co-develop culturally-specific tobacco education workshops for the communities they serve.

Project Access Now
If you can’t pay for your premiums or out of pocket costs, we directly refer people to Project Access Now, which provides OHP assistance partners with us on events and workshops.

NW Oregon Health Preparedness Organization (HPO)
Part of the Regional Disaster Preparedness Organization (RDPO) the NW Oregon HPO is our primary partner for coordinating regional planning efforts and helping ensure clinics are prepared to respond to large-scale health emergencies.
