What We Do


CCHC works at the intersection of community health, systems change, and equity. We bring people and organizations together to drive system-level change, while also providing direct support that helps people and communities right now.

We focus on achieving racial equity by meeting health-related social needs and transforming the systems that shape social determinants of health—like access to care, professional education, data equity and inclusion, and community safety.

We serve BIPOC communities, LGBTQIA+ individuals, immigrants and refugees, and individuals and families who are low-income, and people experiencing homelessness. Our work is rooted in collaboration and guided by those most impacted by health disparities.

Clinic Support and Learning Collaborative

Through the H.E.A.L. (Health Equity Through Action and Learning) Collaborative, CCHC supports member clinics with shared learning, resource exchange, and technical assistance.

We help clinics strengthen operations, build community partnerships, and better serve communities who are systematically excluded from equitable healthcare. The H.E.A.L. Collaborative also ensures clinics have a collective voice in regional efforts and policy advocacy.

Policy and Advocacy

CCHC is a champion for community health clinics. We advocate for equitable policies that support clinics, expand access to care, and advance health equity across Oregon. Our advocacy brings clinics, communities, and policymakers together to shape a healthcare system that works for everyone—especially communities historically excluded from equitable healthcare.

Through collaboration, training, legislative engagement, and our annual Advocacy Day, we empower clinics, community organizations, and individuals to lead systems change.

Emergency Preparedness

CCHC has led emergency preparedness efforts for more than 15 years, helping clinics and communities across Region 1 prepare for, respond to, and recover from disasters.

As the convener for non-hospital clinics in the region, we provide trauma-informed planning support, real-world scenario trainings, and ongoing education for both providers and community members.

Reach & Impact

  • Led the first 2025 PULSE (Preparedness, Understanding, Learning Safety in Emergencies) Emergency Preparedness Summit for OHA Region 1 Clinics and public emergency partners.
  • We provide region wide situational awareness to clinics regarding hazards and threats to their operations through weather forecasts and operational disruptions.
  • We are a conduit to our clinics receiving emergency preparedness resources during and after a disaster from area emergency management agencies.
  • Collaborated with the Coalition of Communities of Color with Modernize Anti-Racist Data Ecosystems (MADE) advisory.
Community Health Worker Training and Certification

CCHC is an OHA-approved training site offering continuing education and 90-hour CHW certification for community health workers who serve our member clinics and communities across the region.

We use a popular education approach to prepare CHWs to address health-related social needs, navigate systems, and serve as trusted connectors within their communities.

  • Eighty-two BIPOC CHWs graduated between 2024-2025.
  • Fifteen unique self-identified cultural groups.
  • On average pre-training income ranged between $0-$21/per hour and increased post-training between $25-$33/per hour.
Health Access and Navigation Support

Community health workers provide health-related social services and health navigation to vulnerable communities. Through our request and referral form, CCHC connects individuals and families to health-related social needs like healthcare services, insurance benefits, housing and basic needs, and more.

We help individuals and families find health care clinics that meet their needs.

We also attend local and regional health fairs to connect community members with trusted clinics, culturally responsive wellness resources, and enrollment support for public benefits.

Reach & Impact

  • Our Health Navigator and Enrollment Specialist served 259 individuals with one-on-one services in 2023.
  • We supported individuals with OHP enrollment, healthcare system navigation, health related social needs navigation, and outreach.
  • Our Health Navigator and Enrollment Specialist connected with 534 community members, attended 90 events, and dedicated 197 hours to outreach.
  • Our supportive housing community health worker helped 40 individuals remain housed between 2024-2025.
  • In 2023 and 2024, CCHC co-hosted 3 health and wellness fairs with our partners to provide vaccinations, health insurance enrollment, chiropractic care, STI health screenings, food boxes, and more!
REALD/SOGI Data Training and Technical Assistance

CCHC provides REALD (Race, Ethnicity, Language, and Disability) and SOGI (Sexual Orientation and Gender Identity) training and technical assistance to help clinics and community organizations collect inclusive data and provide culturally responsive care.

These trainings are designed to improve data practices, enhance equity in care delivery, and reduce disparities in how services are accessed and experienced.

  • Provide a culturally responsive, trauma-informed, and participant-led curriculum for community health centers and traditional health workers on REALD and SOGI data importance and collection.
  • Provide technical assistance support to community health centers and community partners.
  • Partner with culturally specific organizations to provide training(s) for community health workers.
Tobacco Prevention

CCHC leads tobacco prevention efforts across the region with a focus on education, advocacy, and culturally tailored training. We work with clinics, CHWs, and community partners to reduce tobacco-related harm and address the root causes of health disparities, including systemic racism and targeted industry marketing.

  • Between 2024-2025 we’ve trained 57 clinical and community health professionals on the impacts of commercial tobacco and nicotine product use.
  • Seventeen participants received Advocacy 101 training in preparation for future advocacy opportunities.
  • Participants testified with local and state governing bodies in support of local flavor bans.
