Community partners:
CCHC is involved in a partnership with JOIN’s six community agencies who are working together to support our community members whom transitioning from homelessness to stable housing. The Mobile Permanent Supportive Housing (MPSH) retention team provides wrap around services including case management, mental health, health advocacy and peer support services to individuals and families new to housing.
