CCHC Health Equity Programs
Clinic Learning collaborative:
CCHC’s long-term health equity work includes facilitating bi-monthly equity training for our clinic members. As a coalition we work together to provide care that is responsive to the language and cultural needs of community members.
We know the sustained work and relationship-building it will take to advance health equity at the clinic level. As such, the learning collaborative grounds its work in these goals:
- Share health equity learnings
- Implement health equity strategies in clinics
- Build a sustainable health equity network

Our CHW team is here to help by:
- Receiving referrals from clinic partners and community members.
- Supporting/coordinating care with health systems providers.
- Providing culturally centered education and system navigation.
- Supporting navigation and advocacy for various essential social services and resources (e.g., access to food, housing support)
- Providing additional support/education for individuals/families with chronic conditions

Our tobacco prevention program activities focus on the root causes of health inequities while being explicit about how dominant practices continue to perpetuate inequities and trauma among BIPOC communities. We do this through equity-based education and training, as well as policy and advocacy work.
Culturally Specific Tobacco Training
CCHC has worked diligently to deliver a culturally specific Tobacco training to community members! Our training in partnership with Oregon Public Health Institute called, The Impact of Racism, Stress, and Trauma on the African Diaspora, is a culturally specific training focusing on African, Black, and African American experience with systemic racism and its impact on the health of our community.
Participants learn of the history of systemic racism and its connection to the Tobacco consumption and targeted advertisement in the African diaspora, over decades. Throughout the training Black resistance and Black Joy within the African diaspora is uplifted and celebrated as well.
By the end of the training, participants have a deeper understanding of how systemic racism, stress and trauma impact health and how community members can implement this knowledge for change.
