“Transforming and Strengthening the Local Safety Net”

CCHC’s key benefactors

We are thankful for our funders, our collective community, and to you, our generous donors, who make our work possible.

On August 12, 2021, CCHC has received grant funding to help improve clinical care and reduce health disparities in Black communities in Portland from Hampton Family Foundation of Oregon Community Foundation.  This funding will be very helpful in assisting us to reach our current health equity goals with our member clinics. We would like to thank the foundation for their commitment to ensure quality healthcare for all.
