Advocacy
CCHC foster Collaboration across the Safety-Net Delivery System. We facilitate the collaboration among our clinic members to address their needs in order to better serve the patients. By developing strategic partnerships and coordinating with other services, we advocate for a stronger primary and specialty care delivery network.


Resources
We strengthen Community Clinics by facilitating a collective learning community through peer sharing, networking, and by providing direct benefits to member clinics and their patients.

Information and Referral Services
Comprehensive website, social media, and phone referral services connect community members with Oregon Health Plan Enrollment support, community health clinics, and other social service supports.

Our Programs

Health Equity
Facilitating a learning collaborative of clinics to increase clinics’ capacity to provide culturally and linguistically responsive care, develop health equity strategies with our members and provide resources for our clinics to support and retain a diverse clinic workforce.
The strength of Portland’s community health clinics comes from their diverse array of services and their deep knowledge and history of the communities they serve.

Oregon Health Plan Enrollment
CCHC Enrollment Specialist works closely with community to provide one-on-one Oregon Health Plan (OHP/Medicaid) enrollment help through our partnership with CareOregon.

Emergency Preparedness
Facilitating Emergency planning training for our member clinics and providing all the resources concerning Emergency Preparedness.

Health and Housing
Working as Health Advocate as part of Member of a multi-agency Mobile Permanent Supportive Housing Team (MPSH) in partnership with JOIN to provide retention services to newly housed folks.
